Joint working project summary:
The aim of the Joint Working Project is to support project management of the rollout of the integrated care pathway for COPD management across Hammersmith and Fulham. By implementing full roll-out of the integrated service, NHS Hammersmith and Fulham would be optimising the management of COPD patients in accordance with the National Clinical Strategy for COPD. By supporting the roll-out with project management provision, through investment and the availability of in-house resources, Novartis would be a partner in working towards the goal of a gold-standard, integrated COPD service that could become a template for best practice nationally and internationally. A key ambition of the service is to equip and engage primary care to increase the standard of care from QOF to that enshrined in the principles and recommendations of NICE guidelines.
