Project name: Pooling of resources to improve multiple sclerosis patient management through the provision of multiple sclerosis related medical equipment.
Project Period: May 2017 – May 2019
The principal aim of this Joint Working project is to pool resources to enable the South Eastern Trust to develop their multiple sclerosis service for patients and enable the set-up of a new multi-disciplinary clinic through the provision of multiple sclerosis-related medical equipment.
This Joint Working project entails pooling of resources and expertise to enable the South Eastern Trust to redesign their multiple sclerosis service for patients. This new service will provide additional capacity that will enable patients to be reviewed in a timely manner in accordance with national guidelines as well as rapid access to the multiple sclerosis team in times of greatest need e.g. during suspected relapse.
Expected patient outcomes:
Patients receive their care closer to home
Patients have an increased awareness and understanding of their condition and treatment options
Patients have improved access to appropriate services and medicines as defined by local and national guidance
Patients have rapid access to the multiple sclerosis team in times of greatest need e.g. when experiencing symptoms of relapse
Patients have access to more regular review of their condition in line with national guidelines
