Joint working project summary:
Wet age related macular degeneration is a rapidly progressing eye disease which results in loss of central vision and difficulty with performing activities of daily living. Current management requires monthly assessments and regular eye injections. Most patients with wet AMD are followed up in hospital services. The aim of the Gloucestershire Community Optometrist Project is to pilot a new service model that delivers care in the community for patients with wet Age Related Macular Degeneration (AMD) by providing access to follow-up assessments through Community Optometrists.
Expected patient outcomes for this project:
Reduction in average travel distances for patients
Improved patient satisfaction
Increased number of follow ups performed by Community Optometrists
Increased total number of wet AMD follow up slots within the hospital service
Reduction in overall follow up costs for the stable patients
Start date and duration: April 2013 for 12 months.
